Sub Acute Care

Rose Desert Congregate Living Health Facility provides a continuum of high-quality, affordable post-acute care and rehabilitation to medically-fragile or ventilator-dependent people with complex illnesses and injuries. Our unique service programs combine experience of skilled nursing care professionals and rehabilitation therapists with comfort of a home-like environment designed to meet individual needs of our patients at every stage of their recovery.  

Skilled Nursing & Rehabilitation Care

Contrary to other sub-acute care facilities, Rose Desert Care CLHF provides cozy non-institutional environment, that helps our patients in their rehabilitation process. Rose Desert Care facility features private bedrooms, outdoor activity area, individual therapy, and diet to make our patients feel like they are at home away from home. We are genuinely committed to helping your loved ones to be as comfortable as possible.

Interdisciplinary Treatment

  Our staff has unparalleled expertise in providing high-quality , cost-effective medical management and restorative care to individuals with complex conditions. We utilize an Interdisciplinary Treatment Team composed of physicians, nurses, case managers, social workers, physical and occupational therapists,  registered dietitians, and activity director.
